PROGRAM DESCRIPTION:
Supportive housing program for individuals who are experiencing street homelessness or are subway dwelling and are placed into agency independent apartments. Multidisciplinary staff will include professionals and paraprofessionals. An individual support plan is developed with the tenant with goals and objectives that focus on housing retention , community integration and recovery. Tenants may present with an array of needs that may include mental health, substance abuse and physical disability.
PRIMARY RESPONSIBILITY:
Function as a role model to Housing First clients, staff, co-workers and collaterals. Conduct oneself as a professional at all times, keeping work up to date and interacting with the above mentioned parties in a respectful and appropriate manner at all times. Engage and develop trusting rapport with program clients.
TASKS & RESPONSIBILITES:
1. Provide individualized, ongoing guidance and support to clients with mental health and co-occurring substance abuse disorders;
2. Provide information on the recovery process, relating as appropriate, personal recovery experiences;
3. Maintain primary responsibility and accountability for assigned caseload of clients. “Primary responsibility” means provision of needed services within the housing program.
4. Provide peer advocacy with entitlements and assist clients maintain benefits
5. Development of service plans, with client input, aimed at assisting client to establish and accomplish personal goals;
6. Provision of substance abuse services to clients in need of that service;
7. Facilitate at least 2 addiction recovery oriented groups weekly;
8. Collaborate with case managers to insure individual service plans include substance abuse services as needed;
9. Attend staff meetings, case conferences and staff training sessions;
10. Participate in individual supervision with Program Director or designed;
11. Assist clients with referrals to substance abuse treatment programs (detox, rehab) with the client’s consent;
12. Documentation of all activities performed in the Peer Case Manager Role.
