Square Medical Group is urgently hiring for an energized and organized Case Manager for our Co-Occurring Enhanced Residential Rehabilitation Services (COE-RRS) Program in our Framingham location!

We offer a wide range of services for mental health and substance use disorders with a continuum of care that includes medication management, individual and group therapy, IOP, PHP, day treatment, residential treatment, TMS and neuropsych testing.
Position Summary: Square Medical Group is looking for an energized and experienced Cased Manager to join our new team. Our Framingham program opening this April is designed as a 24-bed intensive treatment program providing a recovery home for adult men. This program is one of our newest models that will offer an extended stay and more robust staffing for residents with co-occurring psychiatric and substance use disorders, offering support for male adults looking to pursue independent and recovery-oriented living. We do this through a thoughtful treatment environment that affords a healthy spectrum of psychiatric and clinical services. These services create a therapeutic milieu resulting in residents gaining greater amounts of self-responsibility and self-efficacy. By addressing our resident’s social determinants of health along with their medical, psychiatric, and addiction recovery needs, they can learn important skills to move beyond their illness, so they can pursue a personally meaningful life!
Qualifications:
- The Case Manager will be able to take direction, be prompt, take initiative, complete tasks as assigned, and be able to work effectively as part of a team.
- Articulate with good communication skills.
- Exceptional time-management.
- EMR documentation talents.
Should have at minimum a basic understanding of:
- · Basic tasks of case management in a behavioral health setting
- · The concept of recovery as it relates to behavioral health
- · Peer support (Recovery Coaching)
- · Wellness and recovery planning
- · Harm Reduction
- · Trauma Informed Care
- · Understanding of Social Determinants of Health
- · Proficient knowledge of community resources in the Greater Boston Area including but not limited to housing assistance, education assistance, primary and medical care, vocational assistance, transportation assistance, and other social services.
- Good computer skills such as: Word, Excel, Outlook, Video conferencing
Responsibilities:
- Assessment of each resident’s case management needs including: behavioral health, housing, financial, health and wellness, education and training, vocational, social.
- Planning: in collaboration with other program staff, develop and implement a plan to address the needs noted in the assessment.
- Linkage: link the resident with any other staff or services, including those outside the program and outside Square Medical Group, to achieve the goals of the plan
- Monitor the progress of the resident and adjust the plan as needed
- Advocate for the resident to achieve and utilize necessary services
- Support the client throughout the case management process utilizing elements of Motivational Enhancement Therapy and a strength-based approach.
- Engage in outreach activities to identify necessary resources
- One on one coaching and support to house residents
- Facilitate residents’ development of a network of sober supports
- Support residents’ participation in recovery-oriented activities
- Model effective group participation and recovery oriented perspective for patients with substance use disorders
- Helps residents identify and access needed resources and recovery supports
- Facilitate community-based groups
- Discharge planning including interface with other Square Medical Group and community resources
- Interface with managed care organizations and health plans to obtain and maintain authorization for payment of health plan benefits
- Participate in staff, community and program meetings
- Participate in regular clinical supervision
- Maintain patient confidentiality and comply with HIPAA regulations
- Collect data, complete reports, and document resident appointment sessions in a timely manner
- Able to handle a high-pressure, fast paced environment
- Be able to confidently organize and manage a caseload
- Able to Problem-Solve
- Possess skill set to present detailed, thorough case management updates in conjunction with the program’s phasing deadlines
Requirements:
- Associate’s degree in psychology preferred
- Associate degree with 1-year relevant experience
- 3 Years of similar experience may replace a degree
Schedule:
- Tuesday through Saturday
- Flexible hours: 8:00am – 4:00pm or 8:30am – 4:30pm or 9:00am – 5:00pm

Pay: $50,000.00 per year
