Provides nursing support and educational services to include blood pressure screenings, coaching, health promotion programs and various on-site care functions for external companies. Collaborates with a multidisciplinary team employing strategies, approaches and techniques to support management of health/wellness issues in a patient-centered, coordinated and accessible approach.
Major Responsibilities:
- Identities clinical, social and safety issues that relate to various aspects of patient care. Collaborates with a multi-disciplinary team members related to patient progress, variances, and achievement of expected outcomes and plan of care.
- Collaborates with patients and families to support the plan of care that includes interventions and treatments designed to improve individual outcomes.
- Establishes and maintains effective communication and working relationships with physicians, leadership and team members in markets and sites. Responds to concerns and provides problem resolution as appropriate.
- Provides recommendations on referrals to physicians and other practitioners for treatment as necessary.
- Provides various educational campaigns for employers.
- Makes referrals as needed to community care and services as appropriate.
- Monitors and maintains accurate documentation of health/wellness management activities.
- Coordinates out of plan patient management when necessary.
Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices.
Education Required:
- Associate's Degree in Nursing.
Experience Required:
- Typically requires 3 years of experience in nursing, and experience in wellness and health promotion, and/or patient education and coaching.
- Ability to work independently in the field.

Physical Requirements and Working Conditions:
- Position requires travel may be exposed to weather and road hazards.
- Exposed to a normal office environment and a variety of work-site environments for program delivery. Work-site environments may contain odors, dust and dirt.
- Frequently transports promotional displays, materials and educational supplies (frequently lifts up to 25 lbs. and occasionally lifts up to 35 lbs. Pushes and pulls up to 30 lbs.)
- Visual acuity required to facilitate review of written documents/computer screens, medical records, and to record information accurately.
- Ability and willingness to work flexible schedule/hours. Hours can vary between 8:00 AM-5:00 per the employer onsite visit schedule.
Additional Information:
- This position will support northeast Wisconsin region (including but not limited to Outagamie, Brown, Manitowoc, and Sheboygan counties) with primary focus supporting employer in Outagamie county. Travel and mileage will be reimbursed.
- The goals of the team include supporting and engaging employees in the program and health/wellness coaching and education.
