The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Reports to the Director of Clinical Services. As a member of the IDG, responsible for the provision of skilled nursing assessment, planning and caring in order to maximize the comfort and health of patients and families consistent with policies and procedures. Identifies patient/family needs for providing supportive care and appropriate services in accordance with orders and the IDG plan of care. Performs audits to ensure effective utilization of medications/supplies/equipment. Promote standards and quality through effective and continuous communication with patients and families. Complies with all laws and regulations, including ACCREDITATION AGENCIES standards.
