Our organization is looking for an Intake Coordinator to join our home health team. The successful candidate will be responsible for managing the intake process for new patients, including verifying insurance eligibility, obtaining necessary medical records, and scheduling appointments. The ideal candidate will have a strong attention to detail and excellent customer service skills, as well as a working knowledge of home health services and regulations.
Responsibilities:
- Develop and maintain a home health intake process.
- Communicate with referral sources to ensure accurate and timely patient intake.
- Maintain accurate patient records and documentation.
- Monitor patient progress and provide follow-up services as needed.
- Assist in the coordination of services for patients.
- Provide education and resources to patients and families.
- Collaborate with other healthcare professionals to ensure quality care.

Pay: $22.00 - $24.00 per hour
