Compensation: $24 to $28 per hour

Position Summary: Home Health Care Inc. is seeking a home health intake coordinator to join our team in Golden Valley! As the Intake Coordinator, you would be handling the admission process for new patients by ensuring the availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling all patient visits within prescribed discipline and visit frequency requirements.
Responsibilities:

Review all incoming referrals by conducting an initial screening to ensure they meet the defined admission criteria

Contact referring locations to gather client medical information and necessary documentation

Conduct screening and informational interviews with patients and their families as part of the admission process
-
Enter client-specific information and communication notes into the electronic medical record system

Track clients throughout their stay under our care, ensuring their satisfaction, and follow-up as needed
Qualifications:

Experience in a healthcare office, ideally home health setting
-
Knowledge of medical/health care and insurance terminology is highly preferred

Experience with home care admissions/intake is ideal

Demonstrated effective verbal and written communication skills

Comfortable navigating and utilizing electronic medical records and computer programs

Maintains a positive outlook and approach to communication with clients, families, and other professionals

Excels at time management and prioritization of the most critical tasks

Upholds a professional and respectful attitude in all aspects of the role
