Program Coordinator (Full-time) (Santa Ana, CA)
POSITION SUMMARY
Reporting to the Clinical Manager, the Program Coordinator is responsible for the clinical, operations of the Peer Mentor, Residential, Outpatient, and Alumni programs. The Program Coordinator will oversee the service delivery of all clinical services, outreach and engagement, screening assessment, and ensuring all county required documentation is completed, including surveys as well as oversee the Alumni Program The position also maintains responsibility for upholding all relevant regulations established by the State, County or Federal Government.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Overall ensures staff implement and follow, all clinical policies and procedures within the Peer Mentor, Residential, Outpatient, and Alumni program.
Works collaboratively with OC Leadership to coordinate Alumni Program, from reviewing candidates to scheduling activites.
Monitors and ensures contract requirements are met as defined by the Alcohol and Drug Program Administration and other agencies as appropriate.
Supports the Clinical Manager and Sr Program Director with ensuring that all required reports for governing or funding agencies are accurate. Submittal of the reports falls on the Sr. Program Director.
Responsible for maintaining relationships with multiple referral sources and coordinating care.
Attend and participates in assigned meetings, conferences, speaking engagements and media appearances as required.
Oversees the Clinical services in Peer Mentor , Residential, outpatient and Alumni services co-leads and participates in case conference, program meetings and staff meetings.
Supports staff with their specific duties and issues written or verbal instructions.
Monitors and conducts care coordination and treatment planning with clients, liaison with community agencies or as applicable regarding client progress.
Reviews and approves all clinical documentation and conducts regular quality assurance and quality improvement reviews to ensure that all appropriate record-keeping is performed on a timely basis and that all governing agency regulations are fulfilled.
Ensures all authorizations have been reviewed, approved and will take the lead to follow up on authorization appeals as needed.
Responsible for the development and coordination of community involvement, outreach and support. Outreach to community agencies to provide education on our services and referral processes.
Carry a caseload as needed, and/or participates in all official functions of the programs including community group meetings, public speaking engagements, and other functions.
Assess and evaluate strengths and weaknesses of staff in partnership with the Clinical Manager; develop methods to utilize and enhance strengths, isolate and minimize weaknesses.
Attends all required staff training sessions in accordance with Phoenix House and state licensing requirements, completes all training on time
Attends and participates in program and all staff meetings
Adheres to and promotes the Phoenix House Mission, Vision, and Values while acting as a role model for others
Other duties as assigned
SUPERVISORY RESPONSIBILITIESThe incumbent in this position supervises the Peer Mentors.
EDUCATION/EXPERIENCE/CREDENTIALS
Master's Degree in Social Work, Marriage and Family Therapy or related field
Current registration with the California Board of Behavioral Sciences (BBS) required
LMFT or LCSW with current California Board of Behavioral Sciences (BBS) Registration preferred
Must maintain licensure as a condition of employment.
Two (2) years post licensure highly preferred
Experience in Substance Abuse or Mental Health preferred
Experience with SAPC, DMH, Community Care Licensing, and other governing bodies preferred
Evidenced-Based Trained/Certified in TF-CBT, SS Preferred
Must maintain a valid California Drivers' License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy

Bilingual in Spanish preferred
