Job Summary:
Our QIDP position monitors and directs development, implementation, monitoring and delivery of all individual services to ensure the optimum level of independence possible in the least restrictive setting for our individuals with developmental and intellectual disabilities. Mentors Direct Support Professionals and works in a team environment with other QIDP’s and staff.
Responsibilities and Duties
The primary purpose of the QIDP is to provide coordination and support for the services received by assigned individuals.
The QIDP is responsible for developing and overseeing the delivery of services across settings in order to enhance the well-being, quality of life, opportunities and achievement of personal outcomes for individuals served on their caseload.
Develops, monitors, reviews, and amends person centered Implementation strategies (IS).
Monitor, implement, and/or change any and all programs
Promotes and teaches self-advocacy and advocates for individual
Monitors the health, safety, well-being, and satisfaction of each individual.
Maintain current individual files in accordance with case file organization and retention schedule.
Assures individual’s participation in all decisions affecting his/her life.
Liaison effectively working with ISSA workers, guardians, family members, residential providers, and other stakeholders in order to provide cohesive supports and advocacy for individuals served.
Respond to medical and behavioral emergencies following appropriate health & safety, crisis prevention, OIG reporting, and CIRAS reporting procedures are followed.
Participate in staff training as assigned and maintain continuing education as required. (12 CEU hours annually)
Education, Training, and Experience
At least 21 years old
Possess a Bachelors Degree in a Human Services field
Must meet the qualifications of QIDP. Preferably required QIDP training is already completed and are on the state registry, but willing to train for the right applicant
Valid Driver’s License
One year experience of working with ID/DD population
Must pass a criminal background check
CPR/First Aid/Food Handlers (provided through agency)
Wage: $17-$20 hour based on experience
