Working primarily in the Emergency Department, Inpatient Medicine unit, and/or outpatient substance use treatment program, examples of typical tasks:
1. Engage in timely assessment, treatment, and education and provide interventions for patients who may have a substance use problem
2. Interviews patients with a risk of substance use disorder to assess the need for services; provides counseling and/or makes referrals as required
3. Document findings, interventions and plans in patients' medical records on the same date patients were interviewed
4. Reports suspected cases of child abuse or neglect to the New York Central Registry
5. Provide ongoing support and counseling, family intervention and education about CATCH, ED and SUD services
6. Provide information about community resources and developing appropriate aftercare plans such as coordinating appropriate referral(s) to treatment programs/facilities, including the provision of relevant information about professional services, community resources, and appropriate follow-up communication
7. Participate as part of the multidisciplinary teams in formulating overall treatment and discharge plans; Confers and consults with professional and technical personnel in implementing a multi-disciplinary approach to client/patient care
8. May participates in interdisciplinary Performance Improvement Projects and Complete monthly statistics
9. Keeps electronic records, prepares reports, and related paperwork
10. Escort patients for placement interviews/discharge when appropriate and/or indicated by supervisor
11. Attends meetings, such as staff conferences, social work staff meetings and rounds; participates in work of selected staff committees
12. May supervise graduate and undergraduate social work students in their field placement, as well as volunteers and other auxiliary personnel as needed
13. Perform duties as assigned by supervisor/designee
