Position Summary
Under the direction of the Program and Partnership Manager, this position promotes the optimal health of children ages 0-5 by providing pediatric health education, consultation, training and technical assistance to staff and families at assigned location(s). This position provides limited direct health services.
This position is anticipated to serve roughly 30 hours a week, and candidates can be based anywhere in Oregon as we operate in 13 different counties across the state.
Essential Functions
- Interprets the child’s health status from the healthcare provider to staff and parents.
- Provides health assessments, guidance and consultations for children upon referrals made by staff.
- Develops care plans in collaboration with the health provider so that the child may participate in the program.
- Trains staff on health services to be provided to children such as medication administration and other health services that are identified in the child’s care plan.
- Provides ongoing consultation for sick children to support the control and prevention of communicable diseases.
- Reviews children’s Well Child Exams and Health Histories as needed.
- Conducts health and safety observations and assessments in the classroom in collaboration with staff.
- Provides referrals to community health resources and services.
- May participate in home visits.
- Where applicable, participates in Case Management Team Meetings (CMT) and individualized support plan development (i.e. IFSP/ IEP).
- Documents, tracks, and reports on progress of children with specialized health care needs in the agency’s child and family database system.
- Supports the development and implementation of OCDC’s health policies, practices, guidelines and agency’s goals and objectives.
- Acts as a health and social services liaison with community agencies and partners.
- Assists in situations related to mandated reporting adhering to laws and regulations and adherence to services for foster children.
- Provides training, coaching and monitoring of Safe Sleep practices.
- Supports the development and implementation of program evaluation, monitoring and outcome measurements for children.
- Provides health and wellness promotion.
Education And Experience Requirements
Current Oregon Registered Nurse unrestricted license and a minimum of 5 years of work experience in public health nursing, community health nursing, school health nursing or pediatric nursing and experience working with children ages 0-5.
