Case Manager

Hourly Wage: $21.11/Regular/Full-Time
Manages a caseload of elderly and disabled adults 18 years and older who are functionally or mentally impaired, by assisting them in obtaining necessary in-home services and support in order to remain in their own homes. Assess needs, develop, and monitor implementation of services summary and make necessary referrals. Coordinate with various resource agencies. This position reports directly to the Lead Case Manager. Duties are performed in an office setting and include routine client home visits.
Examples of Work Performed:
Conduct electronic version of CARE comprehensive assessment to determine client's assets, deficits and needs in psychosocial and health areas to determine the need for case management services.
Develops and implements a service plan listing services recommended and describing the division of tasks between the Case Manager, support network and the client; ensures client is referred successfully to all resources listed in the plan and follow-up to see that resources are obtained in a timely manner, appropriate to need.
Review the service summary to document significant changes in the client's condition or situation.
Determine whether resources are appropriately meeting the needs of the client and institute case coordination with community contacts and resources to ensure coordinated service delivery.
Authorize services available in the clients Care Plan and authorize payment for services utilizing the ProviderOne payment system.
Access and utilize the following DSHS data systems: Automated Client Eligibility System (ACES), the DSHS Barcode system, the ProviderOne payment system, the Independent Provider One system, the Washington Connection benefit portal and the CLC/GetCare System.
Maintain ongoing contact with client to enable a prompt response to changes in living environment.
Provide termination planning when the client's informal support systems are operating adequately to meet client need or the client no longer meets case management criteria.
Attend Case Management staff meetings, case reviews, and inter-agency staffing, etc.
Maintain accurate, up-to-date, and complete records of each client. Includes but not limited to annual reassessments, interim assessments, significant change assessments, Service Episode Record (SER) notes, collateral contracts, financial documents, and Consent forms.
Perform in the capacity as Fair Hearing coordinator for other Case Manager in the department.
Provide other support functions (e.g., client advocacy, assistance, consultation, networking, and family support and crisis intervention). Performs Information and assistance activities. Conducts follow-up with clients and/or family as required. Access the CLC/GetCare System in order to manage record, and report service provision and utilization, demographic, resource directory, and consumer website information. This may include accessing the Washington Connection portal to assist individuals in completing an online application for federal, state, and local benefits, services and assistance.
Participate in community liaison activities, which include, providing information services regarding the elderly to service group's senior centers and gatekeepers.
Other duties as assigned.
Knowledge, Skills and Abilities:
Knowledge of the principles and practice of social work.
Knowledge of the aging process, Washington State Aging Network service delivery system, and other service delivery systems.
Participate in training designed to upgrade skills and expand fields of expertise.
Ability to understand and apply federal, state, and tribal policies & procedures and regulations applicable to the assignment.
Ability to adhere to confidentiality requirements of client records including federal, state, and Tribal
confidentiality policies.
Ability to utilize personal and laptop computers, telephone, copy and fax machines, typewriter and adding
machine. Proficient with Microsoft Word/Excel and Access.
Ability to conduct assessments remotely, including the home of the client. Interview clients with Limited English Proficiency.
Ability to communicate effectively both orally and in writing in English and Spanish with client and professionals in the field of aging and long-term care.
Ability to build effective working relationships with co-workers, partner agencies, contract providers and the general public.
Ability to defuse a hostile situation.
Ability to follow supervisory directives.
Ability to multitask in an environment of change.
Ability to establish and maintain effective working relations with peer groups, the general public and tribal officials.
Ability to use crisis, stress and time management strategies for self and clients.
Minimum Requirements:
Bachelor's Degree in Behavioral or Health Science, Social Work or related field.
Two years of experience in Social Service.
Applicant must complete and pass a DSHS Background Check.
Must have access to guaranteed, reliable transportation as required to perform the essential functions of this position.
Complete mandatory Comprehensive Assessment Reporting Evaluation (CARE) training and ADSA New Employee training
Complete DSHS HIPAA Privacy Training online annually.
Complete DSHS IT Security Training for Contracted Staff online annually
Complete ADSA Voter Assistance Training online annually.
Complete online Criminal Justice Information System (CJIS) Security Awareness Training within six months of hire and every two years afterwards.
Must pass a pre-employment background check.
Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.
Required to pass a pre-employment drug test.
Preferred Requirements:
Master's degree in behavioral or health sciences and one year of social service experience.
Experience working with elderly or disabled.
Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
