As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.
Essential Duties and Responsibilities:
- The PRP case managers are responsible for managing a caseload of children, adolescents, and adults.
- Implementation of the individual's Psychiatric Rehabilitation Treatment plan.
- Provide care coordination that includes developing daily living skills, increasing community integration, and helping clients meet rehabilitation goals (such as budgeting, medication compliance, housing, etc.)
- Develop and maintain relationships with other providers in the community
- Attend meetings and collaborate with treatment teams.
- Complete documentation in Electronic Health Record to include daily contact notes, assessments, and monthly summaries as required.
- Provide group and individual services within the facility, community, and/or client’s home based on their personal preference.
- Option to facilitate psychoeducation and skill building groups.
- Other duties as requested by supervisor.
Qualifications:
· High School Diploma or GED. Associate or bachelor's degree in a related field is preferred.
· At least 1 year of experience working with clients with severe emotional and/or psychiatric problems.
· Ability to travel up to 50% of the time
- Ability to use a computer, phone, and electronic health record.
- Have a Maryland State driver’s license,
- Have a reliable vehicle and proof of insurance.
- Be comfortable meeting clients in their home and community.
- Strong time management and organizational skills.
- Willingness to travel between program sites, client homes, and community resources as needed.
Job Type: Contract
Pay: $875.00 - $3,000.00 per month
