Sound Wellness is actively seeking an experienced PRP Admin Specialist (10 to 15 hours)/PRP Counselor (25 to 30 hours) to fill a pivotal role within our organization.
Primary Responsibilities:
Collaborate closely with the PRP Specialist to coordinate patients' schedules and schedule appointments.
Conduct a minimum of six (6) monthly visits for adults and four monthly visits for children.
Be available to work in the office for administrative tasks (10 to 15 hours weekly).
Engage in a combination of onsite (office visitation) and offsite (home/community visitation) services.
Record monthly notes summarizing goals addressed and clients' progress, ensuring alignment with presenting problems and overall mental health needs.
Review and update Individual Rehabilitation Plans for clients every 90 days.
Participate in monthly supervision sessions with the Direct Supervisor.
Attend monthly staff meetings and scheduled trainings facilitated by the Direct Supervisor or Program Director.
Collaborate with therapists and document monthly communication to maintain goal continuity for each client.
Provide guidance to clients on vocational, educational, family, social, and daily life choices.
Actively participate in linking clients to relevant resources.
Demonstrate proficiency in completing the DLA-20 assessment tool.

Salary: $20.00 - $22.00 per hour
