We are currently Looking for NYS Certified HHAs for the communities of Rockland and Orange Counties to work Mondays - Fridays from 8:30 AM to 4:30 PM
Salary for this position is $18.00/hr. This is a full-time, benefit eligible position. Qualifications:
Certified as a NYS home health aide and successful completion of ongoing competency testing
Working knowledge of basic first aid
Basic understanding and commitment to the hospice philosophy of care
Ability to work as a team member and use supervision effectively
Ability to maintain a high level of confidentiality concerning patient and family information
Ability to relate to a diverse population and workforce in a personable and effective manner
Ability to communicate effectively both verbally and in writing
Must possess computer and email skills
Additional Requirements:
Community HHAs require daily access to an automobile with a valid Driver’s License and acceptable driving record

Responsibilities of HHA will include, though not be limited to:*
Follows Plan of Care established by hospice nurse
Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
Performs health care procedures monitored by hospice nurse (example: reinforce clean dressings, use of bed pan, check urinary catheter, etc.)
Provides for the physical care of the patient, as stated in the HHA care plan, (example: bathing, toileting, skin care, etc.)
Assists the patient with ambulation, transfers and turning and positioning as needed
Turns and positions patients who are unable to assist
Under the guidance and instruction on primary nurse, P.T, O.T., maintains a safe environment for patients and proper use of equipment (i.e. wheelchair, Hoyer lift)
Helps patient in performing exercises prescribed by appropriate professional personnel
Assists community patients in taking self-administered medication prescribed by physician
May plan and prepare meals for patient, feeds patient when necessary
Performs a variety of light housekeeping tasks when necessary which are essential to the patients' health care
Performs washing of patients' clothes as needed
Community HHAs, keeps up-to-date record of activities and the patients' condition to be submitted to the hospice nurse for patients' records
Community HHAs, provide temporary respite for family caregivers during assigned work times
Community Aides may do errands as needed
Attends team meetings as needed
Attends twelve (12) hours of documented training annually that may come from any acceptable source
