Job Title: Pharmacist

SUMMARY: Nutrishare is responsible for a wide range of services to patients with Home Infusion needs. The operation is specialized and responsible for sterile preparations, parenteral nutrition, and supplies for long-term home care patients. Nutrishare provides superior products and clinical services in a time manner by dispensing prescribed medications, drugs, and other pharmaceuticals and supplies for consumer care by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned either verbally or in writing.
- Provide Nutrition Support related information and consults with physicians, nurses and other healthcare professionals as necessary.
- Review prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
- Comprehensively monitor patients receiving Parenteral Nutrition Support (PNS) by reviewing the entire available medical record of the patient in progress notes, MD orders, labs, etc.
- Identify metabolic complications in consumers and take action to resolve them.
- Answers questions and provide information to Nutrishare customers on drug interactions, side effects, dosage and storage of pharmaceuticals.
- Maintain established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
- Enter data such as patient name, prescribed medication and cost to maintain pharmacy files, charge system and inventory.
- Instruct interns and other medical personnel on matters pertaining to pharmacy.

EDUCATION and/or EXPERIENCE: PharmD or equivalent pharmacy degree. College or university program degree or certificate, 5+ years of pharmacy experience.
LANGUAGE SKILLS: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, consumers and the general public.
MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumferences and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. Mental stability and the ability to get along with coworkers are essential functions of the job, without which an employee is not qualified.
CERTIFICATES, LICENSES, REGISTRATIONS: Current Kentucky Pharmacy License with Department of Consumer Affairs, Kentucky State Board of Pharmacy, Kentucky Drivers License and proof of personal auto insurance is required. Verification of continuing education units as required by the state board of pharmacy. CEU’s for Kentucky are required annually.
