Our home visiting Physicians office is seeking a reliable full time, patient care coordinator to join our team.
Job Types: Full Time days/hours: Mondays-Fridays 9am-5pm

Some duties include but are not limited to:
- Contacting patients to schedule appointments with providers.
- Intake: taking in patient referrals, running patients insurance for eligibility & benefits.
- Data Entry: entering patients into EMR system/creating patient charts.
- Give instructions to patients as instructed by Physicians.
- Ensure all related reports, labs, diagnostics, and information is filed and available in patients' medical records prior to their appointment.
- Processing physician orders such as durable medical supply (DME), lab orders, diagnostic orders, and referral for specialists.
- Familiarity with Practice Fusion and medical terminology is a plus.
- Take telephone messages and provide feedback and answers to patients/physicians/pharmacy/home health agency calls, etc.
- All other duties as assigned by clinical coordinator or practice administrator.
Skills: excellent communication skills, excellent phone etiquette, ability to take and follow instructions, ability to follow up on orders and test results.
Computer Skills: excel, word, EMR (practice fusion-a plus)
A plus if you are a Medical Assistant, but not required.
Not a remote position.
