Job Description – Hospice RN Case Manager
Department: Nursing
Reports To: Clinical Director

Position Summary: The RN Case Manager is responsible for providing end of life nursing care and hospice case management to hospice patients and their families. Implements nursing interventions and coordinates the Individual/family interdisciplinary plan of care. The RN Case Manager adheres to the regulatory process required by the State Board of Nursing and Federal Conditions of Participation.

Essential Job Functions and Duties:
● In conjunction with the Interdisciplinary Team (IDT), manage and coordinate the Individual and family/caregiver Plan of Care from referral to discharge.
● Facilitate communication about changes in the Individual’s status among IDT members and the Individual’s attending physician.
● Apply the policies and procedures of Longwood and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing hospice care.
● Plan, coordinate, and deliver direct nursing care to Individuals and families according to the Individual’s need.
● Visit each Individual at least every 14 days in accordance with regulatory requirements.
● Maintain a minimum Productivity Standard according to Longwood Policy. Productive time consists of face-to-face contact with Individuals and families in the form of direct care activities. Productive time also includes coordination of care activities with families, physicians, and host nursing facility partners, such as:
TQMs, at-risk meetings, information presentations, or meet and greets.
● Perform a comprehensive assessment of the Individual/family and develops a Plan of Care and ensures the care plan is up to date and reviewed at every team meeting.
● Notify physician and/or appropriate others of significant findings or changes in the Individual’s physical or mental condition.
● Consult with physicians regarding the care of the Individual.
● Assess Individual needs for necessary therapies (PT, OT, and ST) and consult to assist in quality of life and comfort care.
● Ensure that all care is provided consistently with respect for Individual rights. Report any/all allegations of
Individual abuse, neglect, and/or misappropriation of property in accordance with Longwood policies.
● Carry a nursing bag with appropriate supplies to perform needed assessments and treatments. Wear identification badge and smock and maintain professional appearance.
● Communicate all aspects of Individual care with SNF/caregivers.
● Maintain all Individual privacy and confidentiality in accordance with Longwood policies and HIPAA requirements.
● Complete HHA schedule each week and provide SNF/caregivers with copies of the schedule.
Communicate all schedule changes as soon as they are made.
● Supervise and document Home Health Aide Plans of Care and document supervisory visits.
● Participate in regularly scheduled Longwood IDT meetings to coordinate the care of the Individual and family, exchange information, problem solve, and receive staff support and education.
● Participate in the on-call nursing rotation schedule.
● Make every effort to attend deaths of Individuals to assist and support the family/caregiver.
● Provide accurate and timely documentation in accordance with Longwood Policies and Procedures.
● Update and maintain medical records of Longwood Individuals.
● Maintain a regular program of professional development by attending staff development programs and trainings related to current nursing practices.
● Follow standard precautions and infection control procedures at all times.
● Maintain required clinical competency/license as appropriate.
Job Types: Full-time, Part-time
Pay: $48.00 - $52.00 per hour
