Position Summary
We are seeking a dedicated Care Manager with a strong background in healthcare administration and a passion for improving patient care processes. This role will primarily focus on administrative functions related to complex care management, transitional care, quality management, and care coordination. The ideal candidate will have experience with HEIDS measures, Epic EMR systems, and project management within Federally Qualified Health Centers (FQHCs)
Position Responsibilities
- Lead and support healthcare projects focused on care coordination and quality improvement.
- Maintains timely, complete and accurate documentation using technology-based solutions to facilitate communication among patients and their care teams.
- Collaborate with healthcare providers and other stakeholders to ensure seamless care transitions.
- Collaborates internally with Value Based Services team members on strategic initiatives focused on enhancing the quality and value of care provided by our centers.
- Leverages care management and population health techniques to identify program enhancement opportunities.
- Utilize HEIDS measures and other quality metrics to enhance patient outcomes.
Skills. Abilities and Requirements
- Registered or Licensed Practical Nurse in good standing with the Board of Nursing required.
- 2 years of experience working in a care management setting required.
- Strong written and verbal communication and interpersonal skills and excellent customer service skills.
- Knowledge of healthcare industry, healthcare information systems such as EPIC.
- Ability to work independently and adapt quickly

Pay: $64,000.00 - $90,000.00 per year
