We are looking for an experienced hospice RN.
We are a locally owned and operated hospice company - specializing on getting back to the root of hospice. Really spending more time with the patient and the family/caregivers. Less corporate orientated, patient care is our top priority.
We are looking for compassionate, problem solving applicants. As a case manager you need to be able to structure your day, prioritize and multi-task. You will be responsible to structure a plan of care according to the patient's needs.
Job Description:
· The nurse assumes primary responsibility for the admission of patients into the hospice program. The admission process includes the assessment, planning, and implementation of the plan of treatment.
· Strong Communication with the hospital, physicians and other care providers. Ability to form relationships and maintain rapport with clients, families, and referral sources.
· Responsible for the admission process to hospice services. Assuring/Improving Quality Care: Facilitates the patient and family’s right to receive quality, cost effective care.
· Patient/Staff Education: Demonstrates a commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to meet those needs and achieve positive patient outcomes.
· Leadership/Collaboration/Professional Development: Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care.
· Demonstrates leadership behaviors and is committed to the development of self and others. Hospice experience is s must for this position.
Qualifications:
· Previous experience in hospice and/or home health is preferred.
· Current proof of licensure/registration in the State of California.
· Graduate of an accredited school of nursing.
· Demonstrated independence in providing clinical care.
· Demonstrated time management and organizational skills.
· Excellent oral/written communication and interpersonal skills; ability to comprehend written sentences and paragraphs in work related documents.
· Demonstrated active listening skills.
· Must have reliable transportation, valid driver's license, and the minimum state required liability automobile insurance or access to reliable transportation.
Job Types: Full-time, Part-time, Per diem
Pay: $45.00 - $95.00 per hour
