Schedule: Monday through Friday, Full-Time Days
What You'll Do:
- Align all activities with Elara Caring’s mission and vision.
- Assess physical, psychosocial, and environmental needs of hospice care patients and families.
- Take primary responsibility for your caseload, including planning, coordinating, and evaluating treatment plans.
- Conduct initial evaluations and follow-up visits to reassess patient needs.
- Communicate significant findings and changes in health condition to the Clinical Supervisor and other care team members.
- Revise care plans as needed and ensure all team members are informed.
- Teach hospice philosophy and pain management to staff in various care settings.
- Supervise home health aides and LPNs/LVNs, ensuring quality care delivery.
- Provide support at the time of death and perform bereavement assessments.
- Document medication regimens and update profiles according to procedures.
- Handle patient and family property with care and maintain confidentiality per HIPAA rules.
- Promote Elara Caring’s philosophy and ensure quality of care.
Qualifications:
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100lbs
- Driver's License
- CPR/BLS
- Strong communication skills
- Reliable transportation to perform job duties
- Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse
- Preferred candidate will live in St Clair County
- Preferred to have home care experience
