Position Purpose
The Director of Social Services is responsible for overseeing the delivery of all case management programs and housing services throughout the State, including staff recruitment, training, and supervision, program development, program evaluation, grant administration, budget oversight, as well as community relations and partner collaborations. The position is funded, in part, through grants made available through the Ryan White Care Act, HUD/HOPWA grants, and other City and State funding.
Essential Functions
- Provide direction and oversight for the operations of Social Services programs, specifically medical and case management, insurance coordination, food pantry and housing, and ensure compliance with all federal, state, and local government laws and regulations. Assure compliance with all Vivent Health service delivery standards, Vivent Health’s strategic plan, and agency mission.
- Provide direction for Social Services programs including staff supervision, budget management, client enrollment, productivity standards, revenue generation or reimbursement systems, and contract management.
- Provide direction and oversight to the Vivent Health Medical Case Management Program to aid clients in securing access to medical care and other health services, providing medication and adherence counseling, conducting HIV prevention counseling, and assisting patients in accessing specialty care and other services.
- Provide direction and oversight to integrated Medical Case Management and Housing programs to assist clients in securing a health care payer source, conducting HIV prevention counseling, assisting patients in accessing other social services, and assuring that people living with HIV who are experiencing homelessness are provided housing counseling, financial assistance and referral.
- Integrate all appropriate aspects of program services and initiatives into the operations of the Vivent Health Medical Center to enhance successful access to, compliance with, and retention in care. Provide leadership for Medical Home initiatives to maximize patient participation, both for health outcomes and for revenue generation, including eligible patient identification, patient enrollment, ongoing required activities, and process improvements.
- Develop or revise, and lead implementation of, policies and procedures to optimize efficiency and promote effective operation of integrated services.
- Provide leadership to assure that Social Services programs maintain an environment with the highest quality of customer service and achieve high levels of client satisfaction. Promote Vivent Health’s integrated service delivery system through full collaboration with other Vivent Health departments and key community partners to assure quality service delivery to clients.
- Prepare and manage the annual budget for Social Services programs, assuring that Vivent Health maximizes available revenue from all public and private sources, and that programs operate within set financial parameters.
- Assure timely submission of program work plans, CQI efforts, program evaluation, and program reporting requirements in compliance with contracts.
- Develop and enhance collaborations with community service providers and State HIV/AIDS Program staff.
- Serve as a member of Vivent Health’s senior management team, including participation in government relations and fund development, and attendance at and support of Vivent Health events. Serve as a Vivent Health spokesperson, in collaboration with the Public Relations staff for media and public relations and in other community settings as appropriate.
- Any and all other duties as assigned by supervisor.
Qualifications
- Minimum of 5 years management or administration experience that includes fiscal management.
- Thorough understanding of program development, operations and evaluation with experience and commitment to statewide programming.
- Knowledge of and sensitivity to the HIV/AIDS patient population, and/or an eagerness to learn about this community.
- Adheres to all agency policies including, but not limited to, Employee Handbook, Confidentiality, Healthcare Corporate Compliance Plan, Standards of Conduct, and other policies.
Salary:
Starting at $89,000/annually (Depending on Experience)
