League Education & Treatment Center is seeking a Social Worker - Other Licensed Practitioner (OLP) to provide services in the homes and communities of children and youth (21 years old and younger) who are experiencing behavioral and mental health challenges. These CFTSS services can also be provided as TeleHealth when appropriate and based on the preference of the child/family and the clinical needs of the child. The goal is to restore, rehabilitate, and support a child/youth’s functioning, as necessary for integration as an active and productive member of their family and community.
Duties and responsibilities include but are not limited to the following:
- Provide community-based mental health services to children and family who have difficulty engaging in a formal office setting and can benefit from home and/or community-based therapy and rehabilitative services.
- Provide psychosocial evaluations, intake assessments, medical necessity assessments, risk screenings, safety planning, treatment planning, and crisis intervention and planning.
- Provide individual and family therapy, using evidenced based interventions.
- Maintain accurate and timely documentation in the client’s electronic health record system/Netsmart.
- Maintain a caseload of 20 to 25 clients, with a productivity expectation of 20 kept sessions weekly.
- Work as part of an intradisciplinary team.
- Engage and collaborate with external providers and agencies around treatment planning and ensuring the child’s access to needed resources and support.
- Attend staff meetings, supervision meetings, case conference meetings, training and other program and agency meetings, as directed.
Principal Qualifications:
- Master’s degree in social work, psychology, mental health counseling, or any related behavioral health or human services fields.
- License required (LCSW, LMSW, LCAT, LMHC, LMFT)
- One year of experience working with children and youth with behavioral and emotional challenges preferred.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Excellent verbal and written communications skills.
- Strong attention to detail and highly organized.
- Ability to interact with all levels of employees and represent the Agency with external groups/individuals.
- Proficiency in computer skills, including MS Office Suite (Word, Outlook, etc.)
- Bilingual a plus.
