POSITION SUMMARY
This position will receive and process confidential client information from various sources, including over the phone, via fax, email, and in person, for the Los Angeles Long-Term Care Ombudsman Program. The Data Intake Coordinator will enter data from monthly reports.
PRIMARY RESPONSIBILITIES
- Name and save transfer discharge notices to the appropriate regional folder.
- Keep daily logs current.
- Review source documents for accuracy and correctness.
- Input data into the appropriate data system.
- Generate reports as needed, meeting prescribed deadlines.
- Assist with special data-related projects as needed.
- Ensure that facility lists, including but not limited to those in the state-required data collection system, are up to date and that the database is clean.
- Process and track ombudsman badges.
- Adhere to the consent and confidentiality provisions required by the ombudsman program, including maintaining confidential files and information.
- Attend all meetings regarding the maintenance/revisions of all systems relating to data reporting. Meetings may be internal or external.
- Provide backup support for completing intakes as needed.
OTHER RESPONSIBILITIES
- Assist with administrative duties, including assembling binders or other files.
- Provide backup support in maintaining volunteer files as requested by the supervisor.
- Provide backup support to the agency reception desk as needed.
- Notify management if equipment like fax machines or phone lines needs to be fixed.
ESSENTIAL JOB FUNCTIONS
- Ability to use keyboard and phone to intake information.
- Ability to accept preliminary investigations of reports of alleged elder and dependent adult abuse where the Ombudsman Program has jurisdiction.
- Compile and accurately report statistical data conforming with established policies and procedures using the Ombudsman Data Integration Network (ODIN).
POSITION QUALIFICATIONS
- A minimum High school diploma or GED is required, and a minimum of two (2) years of college is preferred.
- Minimum of five (2) years experience in a data collection/reporting role.
- Working knowledge of Microsoft Office, including Outlook and Excel.
- Demonstrated ability to communicate effectively in writing and orally.
- Proven organizational skills to prioritize work and meet deadlines.
- Detail-oriented to flag errors and question inconsistencies in information.
- Ability to interface/relate well with all levels of staff and with the public.
WORK ENVIRONMENT
The position may be in a WISE & Healthy Aging regional office or the main office in Santa Monica, California.
