A New York City-based health services network is currently seeking an experienced, licensed Registered Nurse (RN) to one of their facilities in the Bronx as their new Stroke & Sepsis Coordinator.

Responsibilities: The Stroke & Sepsis Coordinator (RN) will be responsible for:

Coordination, maintenance, and growth of the Primary Stroke Certification Program

Conduction of the monthly Stroke and Sepsis Program Committee meetings for the evaluation of outcomes, policy, procedure and protocol compliance and development, and review of current best practices

Assisting with stroke and sepsis data collection and analysis of on-going stroke and sepsis data

Oversight and coordination of the monitoring, tracking, reporting, and submitting of stroke and sepsis patient data/outcomes and performance metrics
- Providing on-going staff educational opportunities on stroke and sepsis management, prevention, and best practices with all departments interacting with the stroke/sepsis patient population

Assisting with the development of patient teaching material and collaborating with the Community Affairs Department to promote stroke awareness and prevention programs via community organizations and agencies

Make certain of access to treatment protocols utilized by EMS providers for emergency stroke and sepsis care management

Assisting the Patient Experience and Nursing departments with the evaluation of patient perception of care through completion and analysis of stroke patient specific satisfaction surveys

Functioning as a resource and support person for stroke patients and their families, providing stroke education and guidance

Performing other duties, as needed
Qualifications:
-
3+ years of Clinical Nursing experience in a ICU, ED, and/or Medical / Neurologic Nursing setting

Bachelor's Degree in Nursing

NYS Registered Nurse (RN) license

Experience with the Stroke & Sepsis patient population

Experience in Quality Improvement associated with Stroke & Sepsis Care Management and Data Collection
