RESPONSIBILITIES: Provides and oversees the effective provision of nutritional services throughout the East Harlem Council for Human Services, Inc. (Council). Contributes to performance improvement, staff training and development and other programmatic and clinical functions.
DUTIES:
1. Provide nutritional counseling and educational services to patients of the Boriken Neighborhood Health Center and School Health Program, with particular emphasis on the nutritional needs of persons living with HIV/AIDS, diabetes, hyperlipidemia and other chronic conditions, pregnant and lactating women, children and persons seeking to lose and/or control their weight.
2. Provide educational workshops through outreach to community-based agencies and street and community outreach activities.
3. Monitor meal plans developed for the Council's Senior Nutrition and Bilingual Head Start.
4. Provide training for Council staff.
5. Oversee performance improvement activities for nutritional services and participate in performance improvement process and related meetings.
6. Ensure that necessary written protocols are in place and followed for nutritional services at Boriken and its School Health Program, WIC, Senior Nutrition, and the Bilingual Head Start Program, in accordance with the most current medical knowledge and guidelines, as well as funding mandates.
7. Participate in program development activities, including program planning, proposal writing and interactions with funders and regulating agencies.
8. Monitor nutrition service utilization and documentation of services throughout Boriken and other Council programs.
9. Develop and strengthen linkages with other institutions providing supportive services.
10. Participate in continuing education to maintain a solid knowledge base through in-service presentations at Boriken and other agencies and through formal continuing education activities.
11. Perform other function necessary for the proper care of patients and functioning of the Center.
QUALIFICATIONS:
Current New York State Registered Dietician Professional License required.
Bachelor's Degree in appropriate field. Master's Degree preferred.
Proficiency in eClinicalWorks or other certified electronic medical record preferred.
Excellent interpersonal, written and oral communication skills.
Experience serving people living with HIV/AIDS, diabetes, obesity, hyperlipidemia, and other chronic conditions, pregnant and lactating women and children.
Bilingual Spanish required.
COVID Vaccination Required
Certified Diabetes Care and Education Specialist preferred (Salary upgrade consideration for any candidates who are certified).
