We are looking for full time or part time direct care staff (specifically for 2nd shift M-F and/or 1st or 2nd shifts Friday-Sunday). Direct Care Professionals provide support and supervision to our clients by assisting them with daily living skills & personal cares, promoting community involvement and helping with household chores such as cooking and cleaning. This position offers a team environment while working assigned shifts with other direct care staff.
Some of the duties include, but are not limited to: cooking, cleaning, direct supervision, medication administration, completing goals/outcomes, participating in community activities, attending staff meetings, and other duties as assigned. We are hiring for a variety of shifts including afternoons, overnights and weekends. There are no overnight positions available at this time._ *_
MAJOR DUTIES AND RESPONSIBILITES
- Work all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule
- Assist individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans
- Provide transportation to and from activities
- Assist in the implementation of individual plans for individuals and behavioral plan goals
- Promote participation and involvement in community outings - Ensure each individuals' rights are protected
- Treat individuals with dignity and respect
- Report unsafe conditions, environment, and equipment to the Residential Program Coordinator;
- Follow safety protocols to foster a safe working environment
- Activities of Daily Living
- Administer medications to individuals as assigned
- Observe individuals for any changes in behavior or health that may have resulted from medication administration
- Ensure individuals are dressed neatly and appropriately and are groomed at all times
- Encourage and assists individuals to care for immediate personal needs such as toileting, washing their hands, and eating
- Prepare nutritional meals based on individual needs and preferences
- Encourage individuals to assist in the preparation of meals and snacks as appropriate to their individual plan and behavior plan
- Keeps residence (both interior and exterior) and resident personal space clean, organized, and tidy and encourage them to participation in home beautification and upkeep as appropriate according to their individual plans
- Teach individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan
- Cleaning tasks as assigned and/or associated with client specific activities. (Eg. Client restrooms, dishes, common areas etc
MINOR DUTIES AND RESPONSIBILITIES
- Enter daily notes in therap describing client behavior, mood, activities, and
- Document all program data, including but not limited to: individual plan data, behavior plan data, community integration logs, assessments, individual funds requests, mileage logs, maintenance requests forms, and supply acquisition forms
- Document individuals health (i.e. seizure charts, weight, appointments, fluid intake.)
- Ensure that all documentation is completed accurately and in a timely manner
- Ensure Medication Administration Record (MAR) entries are completed daily
- Complete communication log books during shift
- Clock in and out before and after every shift and submits Leave Request forms when applicable
- Observe individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status
- Report any noted problems to the Direct Manager immediately and complete a written incident report form
- Report all medical related incidents to the Direct Manager and the delegating nurse
- Act as a representative for EHCS in a professional manner when required, to families, service coordinators, State officials, community partners, local businesses, and employers
- Maintain resident confidentiality Communicates with Direct Manager when items related to individuals' personal needs and personal space needs to be purchased Training
- Complete New hire Orientation Complete and maintain all required State mandated 245D trainings
- Complete and maintain required onsite and client specific trainings
- Attend all meetings and training sessions as required by 245D trainings and TYS
- Stay in compliance with required annual trainings
QUALIFICATIONS FOR THE JOB
Education:High School Diploma/GED
CPR/First Aid Certification (Preferred)
Experience:
- Experience working with individuals with disabilities/impairments preferred (1 year minimum)
- 1+ years of customer service experience (preferred)
- Valid driver's license and valid car insurance
- Reliable car that is well maintained and suitable for client transportation
- Ability to pass a criminal background check
- Ability to pass and maintain required training for DHS compliance
- Strong interpersonal skills
- Compassionate about the care and well-being of others
- Prioritize and organize work effectively and efficiently
- Communicate effectively with clients, families, staff and other customers
- Ability to work some weekends, evenings, and holidays
