Responsibilities:
- Provide case management services to individuals and families in need
- Conduct clinical assessments to determine client needs and develop appropriate service plans
- Engage in collaborative efforts with both internal and external professionals in order to effectively coordinate the provision of care
- Assist clients in accessing community resources and support services
- Advocate for clients' rights and ensure they receive the necessary assistance
- Monitor client progress and make adjustments to service plans as needed
- Provide crisis intervention and support during emergencies or difficult situations
- Maintain accurate and up-to-date case records, e.g. treatment plans and utilization reviews
- Facilitate therapeutic group and individual sessions
Skills:
- Crisis Management: Ability to effectively handle crisis situations and provide immediate support to clients in distress
- Behavioral Health: Knowledge of mental health disorders and treatment options, with the ability to address behavioral health concerns
- Crisis Intervention: Proficient in providing immediate assistance and support to individuals experiencing a crisis or emergency situation
- Social Work: Understanding of social work principles and practices, including assessment, intervention, and advocacy
- Addiction Counseling: Familiarity with addiction counseling techniques and resources for individuals struggling with substance abuse issues
- Workforce Development: Knowledge of workforce development programs and resources to assist clients in obtaining employment or advancing their careers
- Motivational Interviewing: Ability to use motivational interviewing techniques to engage clients, explore their motivations, and facilitate positive change
- Mentoring: Ability to provide guidance, support, and encouragement to individuals seeking personal or professional growth
