The Position
Korean Case management and Navigation Specialist will be responsible for supporting Bay Area Korean Infoline (BAKI) and Case Management and Wellness Program. This is a unique opportunity to provide individual assistance to limited English Proficient Korean community members that need community resources navigation, social services, and wellness support.
Bay Area Korean Infoline (BAKI) is the Bay Area’s only Korean language toll-free navigation phone line. We provide information, referrals, and navigation in Korean on various government safety net services in the Bay Area including social services, housing, and health care, etc. Our goal is to help community members navigate services and advocate for themselves in accessing needed services.
Case Management Program provides social services and wellness support to Korean families, adults, and seniors in Alameda County. This is a unique opportunity to integrate individual case management and wellness services that provides holistic and wrap-around support to the most vulnerable group within the community.
Responsibilities
BAKI (Bay Area Korean Infoline) Specialist (50%):
- Ensuring BAKI line calls are answered during assigned time;
- Answer, assess calls to determine service eligibility for in-house services and provide follow-ups for all calls;
- Provide referral and/or resource for those who do not require or who cannot be accommodated by in-house services;
- Maintain up-to-date outgoing messages and troubleshoot phone-related issues;
- Represent organization to callers in a friendly, professional manner;
- Record all calls in database daily;
- Update and maintain community resources (i.e. legal, housing, local social services, mental health, etc.) to remain current regarding information and services available to address clients’ need;
- Ensure client level data be entered into SalesForce;
- Receive training to become Covered CA certified enrollment counselor in the first year of employment;
- Assist other KCCEB programs as needed as directed by Program Director.
Case Management and Wellness Program (50%):
- Provide comprehensive case management services, social services follow-ups, and wellness support to API adults and seniors;
- Provide social services for underserved community members, with focus on limited English proficient and elderly individuals and families;
- Conduct comprehensive bio-psycho-social assessments and intakes with the development and implementation of individualized care plans with assigned clients;
- Conduct outreach and in-reach, including home and community visits, to assess or recruit program participants and build connections with community-based partners (i.e. community-based organizations, faith-based organizations, senior centers and/or housing, etc.) for capacity building;
- Assist in the development and maintenance of the community resources (i.e. legal, housing, local social services, mental health, etc.) to remain current regarding information and services available to address clients’ need;
- Conduct wellness screenings for seniors residing in the Alameda County to assess general health, mental health, safety, and services needs
- Ensure client-level data be entered into the Alameda County Social Services Agency Database System
- Maintain client files in accordance with legal and grant requirements, and complete all documentation in a timely manner;
- Maintain client caseloads of 25-40 per annual basis;
- Receive training to become Covered CA certified enrollment counselor in the first year of employment;
- Assist other KCCEB programs as needed as directed by Program Director.
Qualification
- Bachelor’s (BA) Degree in psychology, community health education, public health with 2+ years of experience, or Associate of Arts (AA) degree with 5+ years experiences in community-based programs;
- Professional proficiency in English and Korean required;
- Understanding of the needs of immigrant and refugee populations with limited English proficiency;
- Willingness to learn new skills and expand knowledge in unfamiliar areas, including mental health and wellness;
- Personal qualities of integrity, credibility, and a commitment to and passion for KCCEB’s mission;
- Strong team player with an understanding of the importance of inter-programmatic and inter-agency collaboration;
- Experience in governmental funding including Alameda County Social Services and/or Behavioral Health Care Agency highly desired;
- Must be a dynamic self-starter with initiative, ability to manage a team with a sense of inspiration, collaboration and humor, demonstrated ability to work collaboratively with other organizational managers and partner organizations and communities;
- Must be organized, attention to detail, and efficient in time management;
- Strong communication skills, including diplomacy and ability to ensure confidentiality;
- Computer and database literacy;
- Valid California Driver’s License, clean driving record and insurable.
Compensation and benefits
Salary range: $60,000-66,000 , commensurate with background and experience. Full time. 100% employer-paid medical, dental, and vision insurance. 401K.

About Korean Community Center of the East Bay
Since 1977, Korean Community Center of the East Bay (KCCEB) has been uniquely positioned to serve the Asian community in the Bay Area. After the 1965 Immigration Act which allowed for an influx of immigrants from Asia, KCCEB was one of five Bay Area organizations developed in the 1970s to support the resettlement and community health of Asian immigrants and refugees. KCCEB has evolved into a unique entry point for Korean and other immigrant and undocumented children, youth, adults, families, and seniors who have nowhere else to turn to for their essential support, including health access, social and legal counseling and case management, mental health, tobacco cessation education, and intimate partner violence support.
The Programs
KCCEB comprises three divisions namely the Community Wellness Program, the Community Health Access Program, and the Immigration Integration Program. The Community Wellness Program addresses the mental health and substance use prevention needs of Asians and Pacific Islanders in North and Central Alameda County, with a focus on monolingual low-income communities. The program raises mental health awareness, reduces mental health stigma with attention to a practice towards holistic healing and culturally-responsive trauma-informed services. The Immigration Integration Program creates access to legally authorized no-to low-cost immigration services that are needed for immigrants to transition into American life and become engaged members of society. The Community Health Access Program creates access to affordable health care and social services including healthcare enrollment and literacy education, social service support, case management and health promotion to seniors, a Korean language information and referral service, and community capacity building through the development of health advocates.
