JOB SUMMARY: The Street Medicine Clinical RN serves as the clinical portion of the street team. They will execute clinical judgement in assessing clients within their living spaces. They will organize and coordinate outreach efforts and will assist the Outreach Director in maintaining procedures, to ensure organizational effectiveness, efficiency, and safety.
The Street Medicine Clinical RN participates in the delivery of excellent medical services in a patient centered medical home environment with an emphasis on the prevention of disease by working under the supervision of the Outreach Director to deliver high quality care to patients. The Street Medicine Clinical RN assists other members of the medical team to ensure adherence to all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements and assists in performance improvement activities within the department(s) to ensure the patient’s care is held in the highest regard and kept at the center of all operations.
DUTIES AND RESPONSIBILITIES:
Administrative Duties:
- Creates and maintains the employee schedule for Street Medicine staff
- Serves as the point of contact for Street Medicine employees to ask questions
- Codevelops and implements policies and procedures
- Participates in quality improvement efforts
- Develop and maintain medical supply inventory procedures for Street Medicine
- Track and manage performance data for Street Medicine
Medication Dispensary Duties:
- Work with Outreach Director to develop, implement, and maintain medication dispensary.
- Dispense medications to patients in secure and confidential management.
- Give over the counter meds on case-by-case basis as deemed appropriate by triaging patient.
- Help patient with medication organization in pill dividers as needed.
Clinical Duties:
- Travels with the Street Medicine CHW to visit patients that are unhoused or temporarily housed as scheduled and PRN
- Triage patient’s health needs and prioritize/counsel patient’s on accessing the appropriate form of treatment needed in the moment. This would include evaluating patient under RN scope and reporting to provider, evaluating vital signs of patient, notifying 911 and emergency services if needed, facilitating telemedicine with a provider and answering assessment and evaluation questions, initiating and transporting individuals to Recuperative Care Center when appropriate, and fielding CC and HPI information.
- Perform routine wound care on clients per provider order and notifying provider when wound status changes.
- Transport and deliver prescriptions from pharmacy to patient when patient is need of receiving medications.
- Work in conjunction with CHW staff to provide resources and education to improve patients access to health and health literacy.
- Chart findings and services provided into the appropriate section of the EHR to maintain an accurate and complete record of the visit.

QUALIFICATIONS:
Education Required:
- High School Diploma or equivalent.
- Successful completion of a Registered Nurse (RN) program from an accredited program or school
- Licensed as an RN in the State of Nevada.
- BLS Certification
- ACLS-preferred
- Trauma Care Certification-preferred
Experience Required:
- At least two (2) years’ experience as an RN particularly in a community health setting

TYPICAL PHYSICAL DEMANDS:
Frequent standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds. Occasional sitting. Manual dexterity. Ability to travel to various locations. Able and willing to work outside during seasonal weather conditions.
