- Supervise 5 Case Managers
- Conduct weekly one-to-one supervisory sessions with assigned Case Managers. Through ongoing case review and supervision, insure that all client case records contain required client information, intake, case notes, referrals, and assessment/reassessment details, in accordance with NYC Aging standards.
- Monitors STARS reports (i.e. Assessment Due Reports to ensure that all assessments, reassessments, etc. are completed on a timely basis in accordance with NYC Aging guidelines).
- Ensure follow up monitoring communication with clients is completed by assigned Case Managers in accordance with NYC Aging guidelines
- Serves a back-up to Case Managers and as requested assigns intake screening of prospective clients, including walk-ins and provides information and referral.
- Maintains a caseload of to be determined cases, fulfilling all Case Manager tasks as described in the NYFSC Case Manager's job description
- Collaborates with community organizations in providing services to clients
- Performs tasks as requested by the Program Director and Director of Programs to support the functioning of the Case Management Program
