Transitional Resources' (TR’s) mission is building better health, stable housing, and a community of support for people living with serious mental illness or a co-occurring disorder. Our programs provide pathways to recovery by offering a continuum of behavioral health services coupled with permanent, supportive housing or residential treatment. The RN will serve as a member of the treatment team and plays a pivotal role in helping clients to achieve their personal recovery goals and increase their quality of life. The RN will be expected to work with a diverse population and must be sensitive to the culture, religion, language, age, gender, and sexual orientation of the population served.
ESSENTIAL DUTIES:
1.) Works with psychiatric prescribers in the oversight of clients’ medications. Maintains the prescriber’s schedule to ensure clients are seen on a regular basis. Phones in medication orders to the pharmacy or enters med orders into the online prescribing system as directed by prescribers.
2.) Provides assessment of clients’ physical health needs as needed and coordinates efforts to get these needs met in the community.
3.) Documents client’s medical conditions in the electronic health record upon intake to the agency and keeps medical information on each client updated.
4.) Maintains the Clozaril REMs database, and inputs required lab data to the site.
5.) Coordinates and manages the schedule for the weekly phlebotomist and inputs lab results to the EHR.
6.) Coordinates with hospital nursing staff as needed to ensure continuity of care.
7.) Motivates and educates clients on the benefits of medication.
8.) Provides direct service to clients and documents services in EHR. May accompany clients to medical appointments to ensure coordination of care.
9.) Provides consultation to other agency staff about medications and other physical health issues.
10.) Assists in effectively intervening in consumer crises, including consulting with agency staff regarding potential dangerous or problematic situations. Contacts or coordinates with Designated Crisis Responders as needed.
11.) Develops healthy and effective working relationships with clients and co-workers.
12.) Coordinates medication orders and deliveries with the agency’s Long Term Care pharmacy. Ensures accuracy of clients’ med packets.
13.) Provides injections of psychiatric long-acting medications for clients at the correct time interval and keeps Excel spreadsheet of injections updated.
14.) Facilitates or co-facilitates therapeutic and educational groups for clients as indicated.
15.) Works as a member of a team with Behavioral Health case managers and assists them as needed.
16.) Works with staff and clients of our 16-bed Assisted Living Facility for clients with severe and persistent mental illness to ensure WAC compliance regarding medication.
RELATIONSHIP WITH COMMUNITY AND CO-WORKERS:
1.) Contributes to cooperative working relationships with clinical teammates and with supervisors.
2.) Presents positive professional image of the agency to referral sources and other service providers and community resources needed by agency consumers.
_ MINIMUM QUALIFICATIONS:_
RN, plus familiarity with adults living with mental health conditions. Experience with serious and persistent mental illness preferred. The position requires state Agency Affiliated Counselor registration, first aid/CPR training, HIV related training, TB blood test, criminal background and fingerprint ID, valid driver's license, an acceptable driving record in order to drive Agency vehicles. Must have reliable transportation. Must have basic computer skills. Must be able to tolerate and work effectively with unruly, unpleasant, or bizarre behavior. Must be able to tolerate possible substance use by clients and support a harm reduction approach to substance use.
PHYSICAL REQUIREMENTS
The requirements described here are representative of those needed to fulfill the basic requirements of the position. Must be able to walk up and down stairs several times a day and lift up to 30 pounds if needed.
COVID-19 REQUIREMENTSAll staff must provide proof of having been fully COVID-19 vaccinated. The agency follows CDC and Washington State Department of Health’s guidelines in its measures to prevent the spread of COVID-19.
Must be able/willing to wear a mask if needed during an outbreak of Covid19 at the program. Must be willing to work with clients who may be positive for Covid19 and may be unwilling to wear a mask.

HOURS:
Exempt position, 36 to 40 hours per week, weekdays. Hours to be arranged with the Chief of Clinical Services.
