Job Description
SUMMARY OF DUTIES AND RESPONSIBILITIES
Under the supervision of the Director or Assistant Director of Care Management, the Community
Specialist will conduct visits to eligible patients or health plan members in homes and other
community-based settings. Engages patients or health plan members by conducting interviews aimed
at identifying eligibility and supporting enrollment in home- and community-based services.
Duties & Responsibilities
1) Conducts visits to patients or health plan members in their homes or other community-based locations to
complete eligibility questionnaires for home- and community-based services.
2) Composes documentation, emails, and other communications that are timely, clear, accurate, and wellwritten.
including but not limited to completed patient questionnaires, and notes summarizing each encounter
with patients or health plan members.
3) Ensures all paper documentation is properly entered into a designated database for record keeping.
3) Maintains professional presentation, and positive and productive interpersonal interactions within and
outside of the organization, ensuring all interactions with leadership, staff/coworkers, patients, health plan
members and community partners are reflective of ICARE values: integrity, compassion, accountability, respect
and excellence.
4) Adheres to all operating policies, procedures and requirements as defined and subject to change by all
applicable federal, state, local, and corporate requirements.
5) Diligently upholds all data privacy, security, and confidentiality policies and procedures in all verbal and
written engagements pertaining to patient’s protected and/or personal health information
6) Proactively notifies direct supervisor when issues arise (e.g., information issues, inability to complete tasks,
etc.) as soon as these occur so that proper intervention and/or de-escalation can be applied
7) Completes all required trainings.
8) Performs other duties as assigned.
Minimum Qualifications
EDUCATIONAL LEVEL:
A baccalaureate degree from an accredited college.
LANGUAGE COMPETENCIES:
Spanish Speaking Preferred
