Job Summary
Qualifications: The individual must be approved by the Virginia Board of Counseling as a Qualified Mental Health Professional (QMHP-C or QMHP-Trainee) to provide Intensive In-Home services.
~Provide intensive in-home, crisis-oriented counseling and case management to participants and their families.
~Act in a professional and courteous manner at all times while performing the duties of in-home counselor.
~Educate the parent/guardian on DSM-5 diagnosis concerning the client’s treatment needs.
~Work with clients to foster the development of new improved coping, problem solving, and communication skills, case management, etc. and any goals as directly related to the client's individualized service plan.
~Develop individualized service plans within 30 days of services.
~Review the treatment plan with the client and parent/guardian regularly.
~ Regularly coordinate and collaborate services with referring agencies, social services professionals, and any provider directly involved with the care of the participant and their families served on at least a monthly basis.
~Implement service plans and monitor progress.
~ Complete progress notes for each session conducted.
~Develop quarterlies reports and discharge reports.
~Collaborate with LMHPs and Clinicians to develop SRA entries for continued stay for participants served.
~ Educate family members regarding existing community services, and advocate for connecting family members to these services.
~Respond or arrange for a response to calls on "family crisis" on a 24-hour, seven days a-week basis.
~Maintain up-to-date client records, and prepare the necessary reports promptly and accurately.
~Participate in clinical supervision sessions.
