POSITION SUMMARY: The Clinical Nurse Specialist provides dynamic support across multiple departments within the organization. The role provides performs Siskiyou Community Health Center’s (SCHC’s) organizational immunization coordination, performs employee health activities, oversees medical infection control processes, and provides clinical supervision for care navigators. This position ensures quality, safety, and compliance requirements are met throughout the organization by demonstrating best practices and supporting nurses and other care team members as part of the SCHC care team. This position reports to the Chief Quality Officer.
DUTIES and RESPONSIBILITIES:
· Meets with new employees at onboarding and follows up as needed to ensure compliance with protocols for employee immunizations and infection control.
o Provides and coordinates needed immunizations for new and existing employees.
o Supports and tracks ongoing vaccines for employees, including annual influenza vaccines and COVID-19 vaccines and boosters.
· Assumes the role of Vaccine Coordinator for Siskiyou Community Health Center (SCHC).
o Acts as the primary liaison with the Oregon Health Authority for the Vaccines for Children (VFC) program.
o Orders and monitors vaccine stock and manages inventory in the ALERT iis system.
§ Performs reconciliation of vaccine doses administered at SCHC in ALERT iis.
o Ensures appropriate training for employees on vaccine administration, safety, documentation and handling.
o Works with managers or other supervisory employees when errors or performance issues are identified.
o Works in partnership with the the Operations/Medical Teams to ensure that initial and ongoing vaccine training programs support proficiency in vaccine administration.
· Interfaces with the Safety Committee and reviews safety incidents that pertain to employee sticks and/or pokes to ensure proper procedures are followed and working.
· Serves on SCHC’s Quality Committee.
· Develops and implements and organizational infection control program for medical areas, in coordination with the Chief Compliance Officer and risk management program.
o Monitors compliance with hygiene practices.
o Educates healthcare staff on infection prevention measures, or supports and implementation of best practices.
o Ensures organizational policies and procedures are complete and in alignment with best practices.
o Acts as a member of the response team for potential infection threats or outbreaks.
· Oversees and supports the development of consistent clinical practices within the nursing team, in accordance with industry standards and best practices.
· Leads and facilitates SCHC’s nurse practice committee.
· Leads and facilitates nursing skills fairs.
· Assists with Medical Assistant training, as appropriate.
· Maximizes the functionality of expanded roles and responsibilities within the clinical care team.
· Provides clinical supervision to care navigators, including supporting mentoring and staff development.
· Supports the effectiveness of the care navigator role within the organization.
· Oversees nursing records, clinical practices, and documentation.
· Ensures adherence to best practices, professional ethics, and program requirements.
· Travel to other SCHC locations as needed.
· Others duties as assigned.
QUALIFICATIONS, EDUCATION, and EXPERIENCE:
· Experience working in primary care or other similar outpatient setting is required.
· Ability to establish positive work relationships.
· Understanding of cultural and socioeconomic issues of the local community.
· Willingness to engage in building reflective practice (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision.
· Valid Oregon Registered Nurse (RN) license.
· The ability to handle sensitive situations and stress effectively.
· Proof of current immunizations meeting clinic health and immunization standards.
· Ability to consistently exercise independent discretion and judgment, but willingness to seek assistance and/or advice when needed.
· Must exhibit excellent organizational skills, punctuality, and reliability.
· Ability to effectively communicate with clients, coworkers, providers, and others displaying a courteous, professional manner in-person, by phone, and via email.
· Ability to use an Electronic Health Record in accordance with SCHC’s standards for documentation.
· Ability to continually re-prioritize tasks, considering needs of patients and clinic operations.
· Must be able to uphold strict confidentially and HIPAA standards.
· Valid Driver’s License.
PHYSICAL DEMANDS:
· Frequently lifting, bending, reaching, pushing, carrying, sitting and driving an automobile.
HAZARDOUS CONDITIONS:
· Exposure to blood, body fluids, and infectious diseases, and close contact with sick individuals.
· Exposure to volatile or emotionally unstable people.
· Exposure to sharp instruments and needles, hazardous materials including anteseptics and bleach and hot and cold temperatures.
· Prolonged exposure to video terminals.
WORKING CONDITIONS:
· Primarily office and clinic based with occasional off-site work.
· Occasional evening or weekend work may be required.
· Ability to travel between multiple sites.

Pay: $35.00 - $45.00 per hour
