We are seeking RNs and LPNs to join our team! You will be responsible for delivering high quality, compassionate care and healthcare navigation to older adults.
Responsibilities:
- Clinical support to patients to help close gaps in care in between doctor's appointments
- Collaboration with Doctors and Care Teams to establish patient-centered goals and a plan of care to support the achievement of those goals
- Scheduling patient appointments with practice providers
- Performing patient engagement and outreach as well as program enrollment activities
- Educate and update patients on medical treatments
- Assisting patients with medication management (e.g., refill requests)
- Documenting patient engagement in the medical practice EMR
- Managing inbound patient support calls and routing them to the appropriate clinical team members
- Developing and managing enrollment workflows
- Enrollment data management
- Administrative support to leadership team
Qualifications:
- Current, Valid Licensed Practical Nurse or Registered Nurse License (Individual State or Multi-state Nursing License)
- Experience working in a healthcare environment (at least 12 months)
- Demonstrates creative solution-based problem solving, critical thinking skills, and clinical judgment
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Demonstrate effective verbal and written communication skills
- Highly organized with the ability to manage time and prioritize work flow
- Proficiency with Microsoft Office
- Experience working with electronic medical records (EMR) preferred but not required
- Ability to be self-directed and work autonomously
- A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust
- Maintaining knowledge and awareness of organizational standards for chronic and preventative care management
