The Case Service Manager ensures the delivery of quality services through participation in the planning, implementation, evaluation, and delivery of case management services per the Department of Health and Human Services (HHSC), HCS Principles and Billing Guidelines. The Case Service Manager provides assessment, advocacy, case planning, financial planning, service coordination, and monitoring of services for eligible individuals with intellectual and developmental disabilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assess the needs of the person receiving services through completion of periodic assessments & documentation, such as the following
- Case Service Manager will oversee delivery of all applicable program services in adherence with regulatory principles, and state and federal laws (including HIPAA Privacy Laws) concerning confidentiality.
- Conduct routine home inspections (group home, host home, and Supported Home Living) to report any concerns including those related to health and safety as well compliance with HHSC principles.
- Provide support affiliated to supervision of assigned Home Managers, including oversight of service logs, home maintenance, individual monthly supervision, ongoing training and skills development for residential service providers and consumer safety issues.
- Identify Abuse/Neglect/Exploitation and follow proper procedures for reporting.
- Maintain ongoing communication with local, state and federal entities, including but not limited to the Local Authority, guardians, LARs, family members, Host Home Providers, and a wide variety of professionals in the community.
- Cooperate and assist, as needed, with legal status assessments including guardianship and advocacy needs.
- Monitor trust-fund requests.
- Update and monitor information for the people on assigned caseload
- Review and follow-up on incident reports.
- Make chart audit correction.
- Respond to emergency situations involving people on assigned caseload.
QUALIFICATIONS A college degree is preferred but is not required. If no degree, a certificate recognized by the state as the equivalent of a high school diploma with at least two years of experience serving people with developmental disabilities.
- Excellent verbal skills are essential to communicate clearly with clients and staff members.
- Ability to understand and follow directions from supervisor and the ability to gain comprehensive knowledge of regulations and laws.
- Ability to safely and accurately assess and care for people with medical and mental disabilities and to recognize and report any noted concerns.
- Must be able to adjust in changing priorities and the various details of the job.
- Must be self-directed with strong organizational and time management skills.
- Must have reliable transportation.
- Must be proficient in Microsoft Computer Applications.
- Have a basic understanding of developmental disabilities, knowledge of waiver & non-waiver Medicaid funded programs, as well knowledge of behaviors, medications, special funding sources, learning theory, community resources, community integration practices, normalization principles and rehabilitation sciences.
- Be available as need on call for emergency situations arising from clients on your caseload.
