Case Manager Position
Utah Case Management is looking to hire individuals who have experience as a Case Manager who have worked with low income, homeless individuals and justice involved individuals. The Case Manager assists with housing related services for the client through Utah Medicaid’s Target Adult Medicaid and Housing Related Services and Support program. We offer a flexible schedule allowing you to govern your own hours. This is a hybrid position with remote documentation and in person client visits. We are looking for dedicated employees to assist with services for our clients. We are looking for Case Managers in the Utah County area.
Responsibilities:
- Managing clients services- weekly face-to-face contact with clients, assessing clients needs, coordinating services for clients, etc.
- Documentation- completing, updating, submitting, and compiling necessary documentation for clients.
- Regularly meeting and communicating with clients at their location (facility, etc.)
- Communicating with other case managers to coordinate clients’ services.
- Work with clients to find and maintain housing
- Build and maintain relationships with clients and community partners
- Regularly attend staff meetings and company trainings
- Able to use your own devices
- Able to travel to visit clients
Qualifications:
- Utah Resident only
- Certified Case Manager is preferred
- Must have 2 years of experience working with low income, homeless individuals is required
- Current SSW license in the State of Utah is preferred
- Bachelor’s Degree in human services or related field is preferred
- Good at maintaining relationships with clients and facilities
- Detail oriented
- Self-motivated
- Able to work independently and as a team member
- Good communication and organizational skills
