Job Overview: Housing Coordinators assist individuals with disabilities in locating and maintaining adequate and affordable housing. They identify barriers to housing and assist clients through the process of relocating to community-based housing, providing needed support and advocacy along the way.
Responsibilities:
- Manage a caseload of 8-10 active transitional clients.
- Complete case notes daily, ensuring they are professionally written.
- Identify and assist with resolving barriers to accessing housing.
- Conduct housing assessments and develop support plans for housing stabilization services.
- Conduct housing searches and build relationships with landlords, property managers, and owners.
- Conduct in-home visits and remote work with clients on a regular basis to address progress and provide support.
- Collaborate with the managing team, contributing ideas and feedback.
- Build knowledge of community, county, and state resources for additional income assistance for clients.
- Support clients in applying for all benefits that will help them afford their housing.
- Communicate effectively with case managers, providers, family members, and others from diverse backgrounds.
- Navigate Minnesota Healthcare programs and processes.
- Maintain files and program records.
- Comply with HIPAA and Data Privacy Practices.
Minimum Qualifications:
- Must reside in the Twin Cities or surrounding metro area to travel and meet with clients in the community.
- Reliable vehicle with insurance, a valid driver's license, and driving history.
- Professional references from previous employers.
- Ability to adapt easily, communicate with the team, follow company standards and protocols, work independently, and manage time productively.
Desired Qualifications:
- High School diploma required.
- Associates degree preferred.
- Experience in providing direct social services preferred.
- Knowledge of community and housing resources.
- Experience assisting individuals with mental and/or physical disabilities and socioeconomic barriers.
Work Schedule: This job is flexible. Complete 4-5 hours of work for each client at your convenience, as long as the notes are done by the end of the day. The job is hybrid, with the majority of the work being remote. In-person visits with clients are required once a week.

Pay: $19.00 - $20.00 per hour
