PURPOSE OF POSITION
Under the direction of the Director of Care Coordination, the Supervisor, Care Coordination will structure and lead daily activities for an assigned team focused on serving patients with a complex array of medical, behavioral health and social needs (e.g., Health Home Plus, Assisted Outpatient Therapy (AOT), Health and Recovery Plan (HARP), HIV+, etc
Duties & Responsibilities
AREAS OF RESPONSIBILITIES
Under the direction of the Director of Care Coordination, the Supervisor, Care Coordination will structure and lead daily activities for an assigned team focused on serving patients with a complex array of medical, behavioral health and social needs (e.g., Health Home Plus, Assisted Outpatient Therapy (AOT), Health and Recovery Plan (HARP), HIV+, etc.)
- Provides clinical supervision for care coordinators focused on connecting enrolled patients to needed outpatient services (e.g., primary care, behavioral health, social, home and community-based services), demonstrating reduction of unnecessary emergency department (ED) and inpatient (IP) utilization and improved health outcomes. Participates in staff or patient conferences and other social work meetings. Provides telephonic afterhours coverage to address patient needs and escalates cases as needed.
- Oversees the provision of care management services to all patients eligible for the New York City Department of Health and Mental Hygiene behavioral health grant services in accordance with the federal Office of Planning, Research and Evaluation (OPRE) requirements, including but not limited to monitoring program budget spending, collaborating with single point of access (SPOA) on program census, reporting, ensuring service provision, and site visit preparation and facilitation.
- Develops and maintains expertise in all policy and procedures (P&Ps) specific to the operation and oversight of the Complex Care/Behavioral Health and HIV+ patients (e.g., Health Home Plus, HARP, BH grant programs, AOT, etc.); and supports program adherence to all regulatory requirements.
- Leads and co-leads regularly scheduled individual supervision and team meetings with assigned staff; maintains supporting documentation of such meetings. Verifies that supervisory activities are focused on division priorities, strategic pillars and performance metrics. Provides mentoring and coaching to support staff development and overall performance improvement of key metrics, including patient engagement and enrollment targets.
- Supports effective completion of patient incident reports, closed-loop communication and coaching of staff on areas for improvement. Interprets and communicates procedures, regulations and program directives to staff, patients, care team members and other stakeholders.
- Utilizes reports to support population management and staff awareness of patient needs. Monitors and responds to critical alerts of ED and IP admissions/discharges, incarceration, etc.) through proactive and timely communication and follow-up strategies. Provides support/guidance for responding to patient crises. Ensures that follow-up services adhere to established P&Ps and identified best practices. Participates in interdisciplinary meeting and conferences as needed.
- Partners with the Director of Care Coordination to develop, implement, test, and assess quality assurance and performance improvement plans (e.g. Plan-Do-Study-Act) that are designed to support achievement of programmatic goals and benchmarks. Conducts chart reviews on a regular (at least quarterly) basis and facilitates closed-loop communication and coaching of staff to foster performance improvement.
- Participates in staff recruitment, orientation, and training activities. Leads staff performance evaluations and institution of progressive discipline as needed. Maintains positive and productive relationships with staff, colleagues and stakeholders that reflect professional, courteous and respectful engagement. Supports and implement activities designed to enhance and maintain staff morale.
- Demonstrates integrity, compassion, accountability, respect and excellence in all interactions with leaders, colleagues, patients and community stakeholders. Adheres to all operating policies, procedures and requirements as defined and subject to change by all applicable federal, state, local, and corporate requirements.

Minimum Qualifications
Qualification Requirements:
1. Master’s degree from an accredited college or university; and one (1) year of administrative and operational support or related experience; or
2. Baccalaureate degree from an accredited college or university; and two (2) years of experience, as described in “1” above; or
3. A satisfactory equivalent combination of training, education, and/or experience, in which each year of relevant training and/or experience can be substituted for 30 semester credits from an accredited college or university. However, all applicants must at least have a High School Diploma or its educational equivalent.
4. All employees must complete the NYC Health + Hospitals Supervisory training within six (6) months of appointment.
Department Preferences
QUALIFICATIONS FOR THE JOB - CERTIFICATION(S)/LICENSE(S):
Licensed level healthcare professional with prior experience in a behavioral health setting; OR Master’s level professional with two (2) years prior supervisory experience in a behavioral health setting.
EDUCATIONAL LEVEL: Master's degree in social work, Mental Health Counseling, or related field.
KNOWLEDGEABLE IN: Health Home care management or other care management, transitions of care, or care coordination models including for individuals with serious emotional disturbance or serious mental illness. Creating and utilizing reports to impact quality assurance and performance improvement. Willingness and ability to regularly travel between assigned work sites.
YEARS OF EXPERIENCE: Minimum of 2 years of experience in care management, care coordination, or case management. Minimum of 2 years of supervisory experience in a behavioral health setting.

Pay: $75,065.00 - $86,325.00 per year
