General Statement of Duties
The Team Lead works along-side the Residential Manager to develop, implement, and coordinate supports that address the needs and interests of consumers and assist them in safely reaching their personal goals for skill development and community integration.
Essential Job Functions
- Provides direct care when needed to ensure that consumers get to medical appointments, programs and activities and receive daily care as intended.
- Schedules medical appointments for consumers and accompanies them; ensures emergency care is received as needed.
- Conducts regular consumer meetings.
- Prepares menus and ensures dietary needs are met.
- Provide direct care and support to individuals with disabilities or special needs
- Assist with daily living activities such as bathing, dressing, and grooming
- Administer medications and monitor health conditions
- Implement behavior management strategies and provide emotional support
- Assist with meal planning, preparation, and feeding
- Support individuals in participating in recreational activities and community outings
- Document individual progress and report any concerns or incidents to the supervisor
- Ensures fire and emergency drills are conducted as required.
- Ensure new staff receives on-the-job orientation and training
- Routinely meets with supervisor and participates in management meetings and activities.
- Maintains positive relationships with persons, businesses and organizations that seek to work with consumers.
Qualifications:
- Must be 21 years of age or older
- High School Diploma or GED
- Two (2) years experience working with persons with physical and/or developmental disabilities; 1 year experience as supervisor
- Ability to complete certification as a Safety Care Specialist; certification requires physical moves that include, but are not limited to, kneeling, bending, pushing, pulling and supporting another’s weight
- Ability to understand and implement individualized supports
- Ability to effectively communicate with the consumers with whom they are working
- Ability to create appropriate activities for consumers
- Strong written and verbal communication skills
- Ability to effectively complete work-related documentation through paper and computer records system
- Computer literacy in using Word, Excel, Outlook and Electronic Case Records systems
- Valid driver’s license and abstract (not to exceed 5 points)
