Job Summary: The Counselor oversees the well-being of patients during treatment, requiring a comprehensive understanding of their substance abuse history, empathetic support, and a commitment to aiding them in recovery. Duties involve offering guidance to stabilize their lifestyles, providing case management services covering various areas such as housing, employment, legal matters, and childcare. The role entails adhering to regulatory standards and program policies while addressing patient concerns and facilitating their rehabilitation journey.
Responsibilities:
- Maintain accurate patient records and stay informed about relevant treatment protocols and guidelines.
- Conduct individual and group counseling sessions, scheduling appointments promptly and documenting progress.
- Develop and update treatment plans, conduct psychosocial evaluations, and vocational assessments.
- Make appropriate referrals for education and employment opportunities.
- Ensure follow-up on medical appointments and prescriptions, coordinating with medical staff as needed.
- Assist patients in accessing public assistance and monitor their welfare history.
- Handle administrative tasks including paperwork completion and HRA forms.
- Coordinate requests for medication pickup and group counseling sessions.
- Attend meetings and conferences, addressing any client issues with supervisors.
- Monitor and document urine test results, ensuring proper collection procedures.
- Stay updated on specialty area knowledge and participate in agency projects and initiatives.
- Adhere to procedural guidelines outlined in the procedural manual.
