Developmental Pathways (DP) has an opportunity for a Senior Case Manager (SCM) - Active in our Case Management department. This role will be responsible for verifying eligibility and developing, monitoring, and assuring implementation of the Service Plan (SP). The SCM Active will manage a caseload of individuals receiving waiver services and maintain documentation needed quarterly and annually. This role will also facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs, ensure appropriate interaction and communication, and implement IDT meetings as appropriate to modify services. In addition, this role will provide case management services and support to temporarily assigned individuals, support with department and team coverage as needed, and provide training and mentorship for Active Case Managers.
This is a 40-hour week, hybrid position, working remotely and in-person, and coming into our headquarters office in Aurora, CO, and/or other offsite locations as needed.

Requirements
-
Bachelor’s Degree; or
-
Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities; or
-
Some combination of education and relevant experience appropriate to the requirements of the position

Must have demonstrated exemplary performance while working within service coordination or case management positions, including the following areas of performance: demonstrated initiative within the department or agency on projects, demonstrated leadership skills, adaptation to change, and problem-solving skills.

Valid Colorado motor vehicle license & proof of motor vehicle insurance

Ability to meet and maintain agency driving requirements
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
Preferred

Knowledge of rules and regulations for Health Care Policy and Financing and Colorado Department of Public Health and Environment

Two (2) years of experience in service coordination/case management at Developmental Pathways or similar agency

Familiar with Microsoft Office (Excel, Word, Outlook, etc.) and be able to use these and other required programs at an intermediate level
