Position Description:
Provide Social Work services to older clients and families in accord with the standards of the social work profession and requirements of the funding source to those 60 years and older residing in Spring Creek Towers allowing them to successfully age in place. Provide services to large number of Russian-speaking clients of the NORC SSP but not exclusively.
DUTIES & RESPONSIBILITIES:
Interview clients, conduct NORC Assessment, develop NORC plan of care, monitor implementation and adjust plan as required by funding source and adhering to standards of the social work profession;
Provide case management and case assistance services in accordance with standards of the social work profession and grant requirements and guidelines
Provide group social work services as determined by needs of program and under supervision of Executive Director of the NORC
Work closely with NORC Nurse and provide documented evidence of interdisciplinary care planning required
Conduct Health Indicator surveys as indicated by Executive Director and as required by funding source. Use results of surveys to guide practice as indicated by Executive Director and as required by funding source.
Refer clients to community resources as indicated by assessment of needs and NORC Care Plan
Provide services in Russian as required
Work with the Executive Director to develop procedures, schedule workflow, assignments and ongoing programs assuring that these comply with all contract, grant, nonprofit guidelines and social work professional standards
Work with the Executive Director to develop programs based on documented needs of the community of seniors
Work with Executive Director to investigate and develop resources to meet specialized needs of groups in community
Maintain accurate records and provide timely statistical and activity reports. Assure that information is up-to-date and assessed on an annual basis. Client confidentiality agreements and Release of Information forms renewed annually.
Record work in the STARS data base and any other data base which may be required;
Assure contracted levels of units for responsible areas are achieved each month.
Comply with all present and future requirements of contracts and grants
Participate in case conferences and supervision sessions; work as part of interdisciplinary team to provide coordinated services to seniors; present cases during case conference meetings and adjust care plan as indicated
Participate in outreach, educational and health promotion events to assure client participation and assistance;
Active participation in supervision sessions with supervisor
Participation in continuing education required
Remain current in general office technology
Provide supervision of MSW-level student interns in accordance with requirements of the social work profession and the requirements of their respective graduate schools. Maintain field instructor certification.
Perform all other duties as assigned.
QUALIFICATIONS AND SKILLS
Graduate of an Accredited College or University with a Master of Social Work (MSW) degree required; LMSW preferred.
Strong commitment to mission of Spring Creek Senior Partners NORC SSP
Experience in a nonprofit environment.
Experience working in hospital setting preferred
Nonjudgmental, socially perceptive, empathic and a good listener.
Sensitivity to and understanding of the needs of the elderly. Prior experience working with seniors desirable.
Demonstrated knowledge of social theories, practices and guidelines of the social work profession; Dedication to core competencies of social work profession

WORK ENVIRONMENT
This job operates in a professional office environment, periodically at nearby community center and other locations within the Spring Creek Towers complex. Occasional meetings in other boroughs of NYC.
Case load is heavy, work is intense, often crisis oriented, and unpredictable; many clients and families have multiple and complicated situations.
Assessment and services provided to homebound seniors in their homes
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines, etc.
PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel and reach with hands and arms. The employee must frequently lift, carry and/or move objects up to 10 pounds and occasionally lift, carry, and/or move objects up to 25 pounds. The employee must frequently be able to walk up and down approximately 7 stairs in the office as well as walk from the office to apartments or other locations around the housing complex when conducting home visits or other activities. Meetings in other boroughs or locations outside the complex may require use of public transportation.
TIME COMMITMENTS
Hours are generally Monday through Friday, between 9:00 am and 5:00 pm
Must be available for special events and trainings.
