Essential Job Functions:
Provides clinical social work services to patients and their families. Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems.
Coaches patients on how to manage ongoing health conditions and provides therapy to help patients make positive behavior changes.
Perform outpatient psychotherapy and consultations; perform initial evaluations and treatment plans.
Prepare discharge summaries and reports to referring providers.
Read mandatory internal communications distributed by email or hard copy.
Skills & Qualifications:
Excellent analytical and communication skills, oral and written
Ability to establish and maintain interpersonal relationships with others, including patients, members of the integrated care team, and referral agencies/specialists.
Ability to use computer hardware and software for extended periods of time in order to document encounters.
Ability to use electronic medical records. Experience with Athena Health is a plus.
Working Environment:
This position requires the ability to work respectfully with patients from diverse social and cultural backgrounds. This is not a remote position. The job is performed in an office setting in our Mental Health building located at 157 Towne Avenue Plainfield VT. The LCSW is expected to work within the legal and ethical guidelines of the profession. This role routinely uses standard office equipment, computers, Microsoft Office 365 suite, and Athena Health EMR software. Our practice follows the CDC recommendations for required vaccines including boosters. Employees of The Health Center are required to maintain current vaccines based upon the current CDC recommendations. These policies are reviewed annually. Masking policies may be imposed to protect our staff and our patients. All policies in effect are considered mandatory conditions of employment when in effect.
Education, Licensure & Experience:
Requires a master's degree of social work (MSW) and a valid Vermont LCSW Counselor license are required for this position. Participation in continuing education and keeping current on training and research in the mental health field is encouraged.
