The Family Resource Case Manager provides comprehensive services to families experiencing homelessness or low-income families within the San Fernando Valley area. The case manager will be responsible for providing crisis intervention, assisting families with short-term goals, connecting families to resources, following up to ensure families are connected, and working collaboratively with other programs within the agency. The Family Resource Case Manager will be accountable for providing both field-based services and on-campus services to families. The case manager will also be an essential part of the NVCS program department and have an opportunity to help set the direction for family services.
Essential Duties:
· Provide street-based and on-campus case management to families experiencing homelessness or are low-income within the San Fernando Valley.
· Advising in crisis interventions and working with multiple departments and partners to assist families.
· Ensure that families have their basic needs covered and develop an individual service plan for each family to achieve their goals.
· Assist families in obtaining additional services needed, including but not limited to benefits, housing linkages, substance use disorder treatment, and mental health/health services.
· Utilize evidence-based practices such as motivational interviewing, harm reduction, and housing first models.
· Complete follow-up with clients to ensure that referrals are completed and provide advocacy on their behalf if needed.
· Provide support and guidance for families using a client-centered, strengths-based, “do-whatever-it-takes” framework.
· Responsible for reporting abuse as a mandated reporter, which includes immediately reporting any concerns concerning violation of a person’s rights, actual and/or suspected abuse.
· Participates in agency meetings and community meetings as designated by the supervisor
· Additional tasks, projects, and responsibilities as assigned by the supervisor
Qualifications:
· 3 years of experience providing case management services
· Ability to demonstrate empathy and a non-judgmental attitude when engaging with families
· Master’s degree in Social Work (MSW/ACSW/ASW), Marriage Family Therapist (MFT/AMFT), Counseling Psychology, or similar is preferred
· Computer literate; able to effectively use a computerized database for client file management, with basic skills in Microsoft Word, Gmail, Excel programs, etc.
· Ability to authentically build strong working relationships with people of diverse socio-economic and cultural backgrounds to meet our families where they are at
· Bilingual: Spanish/English preferred
· Must have a valid driver’s license and be vaccinated against COVID-19.
· Strong experience working within team environments and ability to collaborate
· Ability to juggle multiple projects and deadlines, thrive on problem solving and challenges
