Enjoy working with a team of health care professionals to give direct nursing care to patients? Full-time LPN position available. LPNs provide skilled nursing care and incidental tasks essential to the patient's care at home. Coordinate services with patients, families and caregivers as well as other members of the home health or hospice team.
- Provide direct quality skilled nursing care including: personal care, administration of oral medication, injections, blood draws and incidental tasks essential to the patient/client's health care at home to prevent or postpone institutionalization.
- Assist families to maintain their normal and usual activities of life.
- See referred patient/clients as directed by the RN.
- Assist patient/client in learning appropriate self-care techniques to maintain as much independence as possible.
- Maintain quality charting and records as required by agency policy and federal and state regulations.
- Report updates or changes in assessments to Registered Nurse (RN) Case Manager.
- Coordinate services and visits with patient/clients and/or family caregivers, as well as other members of the home health team.
- Provide continuous care.
- Participate in on call rotation and related duties.
- Maintain nursing skills through attendance at Annual Skills Fair or other continuing education opportunities.
- Participate in quality improvement (QI) activities.
LPN with one year's experience preferred.
Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager. VNA is subject to the CMS vaccination mandate. Valid driver's license and reliable transportation required for all positions. Mileage paid in addition to wage.
