We are looking for a full-time, Unit Manager to join our Nurse Leadership Team. Duties include organizing and directing nursing services for residents on an assigned unit, the Resident Assessment Instrument/Minimum Data Set (RAI/MDS) process to include assessment, planning, implementation and monitoring of resident care. Responsible for quality and performance in accordance to the Hawaii State Nurse Practice Act.
Applicants for the Unit Manager position must be a Registered Nurse licensed in the State of Hawaii with management experience.
Duties and Responsibilities: Duties are determined by the Company’s needs and may include, but are not limited to, the following:
- Responsible for overseeing nursing personnel and resident care on assigned units in collaboration with nursing leadership team ·
- Reviews and updates risk assessments ·
- Responsible for developing, monitoring, and revisions of the care plans on assigned residents ·
- Ensures that the care plans for all residents on the assigned unit are adhered to and implemented ·
- Collaborates with nursing personnel on revisions to care plans for all changes to residents’ conditions ·
- Collaborates to provide in-service training and huddles to staff on all shifts to care plan changes ·
- Participates in IDT and care plan meetings, Utilization Review meetings and inputs UR notes ·
- Responsible for the review and renewal of the DHS 1147 form for assigned residents as assigned ·
- Collaborates with clinical team to ensure infection control and prevention, antibiotic stewardship program are adhered to ·
- Oversee wound referrals and treatments on assigned units
- Performs daily rounds on assigned units; identifying problems for follow up and collaborates with Resident Care Coordinators and nurse on assigned unit
- Responsible to review and assist with documentation for all residents receiving skilled services to ensure documentation meeting skilled nursing service requirements
- Responsible to oversee completion of all incident reports and final closing of such reports on assigned unit. Oversight on incident reporting and retraining of staff if indicated ·
- Reviews behavior monitoring and psychotropic monitoring for accuracy and collaborates with the interdisciplinary team to create, modify and implement the residents’ care plans
Requirements
- Graduate of an accredited nursing program and currently licensed as a RN in the State of Hawaii
- At least One (1) year as a Registered Nurse.
- Leadership skills preferred.
- Long-term care experience preferred.
- Current CPR Certification required
- Knowledge on the needs and moods of the geriatric resident.
- Ability to enforce work rules and coordinate efforts with personnel.
