Position Title: Transitional Case Manager
Primary Responsibilities:
- Provide case management to residents at transitional housing facility.
- Develop service plans with measurable and timely goals for program participants in collaboration with the medical, behavioral health and case management team, with goals to promote retention in medical care, HIV viral suppression, stable income and a transition to or maintenance of stable permanent housing. The case manager’s focus will be issues related to income, benefits, tenancy skills, financial counseling and legal issues. Provide linkages to supportive services as needed.
- Accompany residents to appointments as needed to provide support, to advocate for residents, and to assist clients with following through with plans for housing, benefits, legal, etc.
- Maintain up-to-date documentation; ensure electronic records are up-to-date to determine continued eligibility and ensure required funder documents are signed and on record.
- Ensure client confidentiality.
Other Duties:
- Assist the Executive Director, Program Director, Clinical Director and Case Managers in their duties as required
- Develop relationships with providers of public benefits (GA, SSA/SSI, SNAP), legal services, and permanent housing programs that transitional housing participants may be eligible for (Rent to Work, PIC’s Coordinated Entry System, rapid rehousing programs, etc) and programs to promote treatment and support services for persons with HIV.
- Represent Gregory House Programs in the community as requested and maintains good working relations with other agencies and persons providing services and support to GHP program participants.
Required qualifications: Associates degree or higher in health related field preferred, however, experience can also be put toward qualifications.
Experience with individuals who have special health care needs, including HIV/AIDS, Hepatitis C, medically fragile, developmental disabilities, emotional, behavioral and/or mental illness, substance abuse, individuals with physical disabilities.
Valid Hawaii Driver's license, clear traffic abstract and willingness to use properly insured vehicle.
Work Hours: Monday – Friday, 9:00 a.m. – 5:00 p.m.
