Responsibilities:
The Disease-Specific Program Coordinator works with physician and administrative leadership to promote optimal patient care for a disease-specific patient population at a site-based level and to collaborate with peers across Legacy Health to promote integration of best practices throughout the organization. Serves as clinical expert resource for staff providing disease-specific patient care. Facilitates team coordination in the provision of evidence-based care. Develops and maintains disease-specific protocols and processes to support provision of care in conjunction with Service Line leadership (if applicable). Supports patient and community education. Prioritizes work and effectively manages time and projects with minimal supervision.
Qualifications:
Education:
Academic degree in nursing (BSN or MSN/MN) required. Internal candidates promoting may meet the degree requirement with verification of bachelor’s or master’s degree in progress with an expected completion date within 2-years of the promotion date; progress will be reported periodically to the site Chief Nursing Officer (CNO).
Experience:
Two years of experience in the provision and coordination of care for the disease-specific patient population required, 3 years preferred. Experience leading performance improvement efforts and disease-specific program management strongly preferred.
Skills:
Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.

Pay Range: USD $51.18 - USD $76.44 /Hr.
