Advancing Communities Everywhere's Community Patient Care Advocate is an integral part of our companies Marketing & Communications and Clinical Departments. You will have the opportunity to network with clients, advocate for client's mental health stability and safety, and interact with community partners and other patient care advocates. You will link clients to appropriate Clinical services that Advancing Communities Everywhere offers , as well as additional MH and Community Resources. You will also act as a liaison between ACE, our Clients, and the community.
Responsibilities of the Community Liaison:
1. Work extensively with Community Agencies, Liaisons, Patient Care Advocates, and Care Coordinators, Managed Care representatives in accessing resources and linking clients to appropriate services.
2. Foster positive relationships between prisons, community service, home, and individuals in the surrounding communities
3. Referral of clients and families throughout the Community for Wrap around service participation
4. Collaborate with community organizations to meet the needs of ACE and clients
5. Interact with our staff and clients to ensure consistent communication
6. Quality Assurance of Clients/QMHP
7. Assist with company Marketing and Relations campaigns
8. Office Based, 40 Hour work week
Requirements of the Community Outreach Liaison:
1. A Bachelor’s Degree or Higher level of education to include but not limited to Social Work, Counseling, Psychology, Nursing, Human Services, Marketing, Advertising, Project Management, etc is preferred but not required.
2. 1-2 years experience with Event Marketing, Customer Service, Call Center, Community Non- Profit Engagement and Public Relations or related professions.
3. 1-2 years experience in Community-Based Services including but not limited to Crisis Stabilization, Mental Health Skill Building, Intensive In-Home, ABA, SA/ARTS, Outpatient, etc.
4. Strong written and communication skills, and Detail Orientation are required
5. Able to work in a fast-paced environment with diverse groups of people.'

- Monday - Friday, 8:00 am - 4:30pm and 10:00 am - 6:30pm
